Statewide Issuance

With the new statewide issuance law now in effect, if the vital record you are requesting falls within the dates listed below, you may obtain copies from any Register of Deeds office in the State of Wisconsin.

Records Available for Statewide Issuance

Birth Certificates – from 10/1/1907 to present

Death Certificates – from 9/1/2013 to present

Marriage Certificates – from 10/1/1907 to present

Divorce Certificates – from 1/1/2016 to present

Tribal Marriage or Divorce Certificates – from 1/1/2016 to present

Records requested prior to the dates listed above can only be obtained from the County in which the event occurred. Divorce records prior to the dates listed above can only be obtained from the State Vital Records Office.

FOR OFFICE WALK-INS: Due to the change in how our office now obtains vital records, it may take 15 to 20 minutes to receive a copy of the requested record in person.

Completed applications for vital records are accepted until 3:45 pm.

Any requests submitted after 3:45 pm will be processed the following business day.

IMPORTANT APPLICATION INFORMATION:

  • Please read the applications carefully to see who qualifies to receive a vital record certificate.
  • If you are NOT a member of the immediate family as listed on the applications, you may only obtain the certificate under the following circumstances:
    • You have a written and notarized authorization, clearly stating the relationship of the authorizing party to the person named on the record.
    • You provide documentation showing a legal purpose for obtaining the record.
  • Requestors must demonstrate direct and tangible interest in the record requested.

Fees

  • First Copy: $20.00
  • Additional Copies: $3.00 each (cash only)
  • Money order or cashier’s checks only for mail requests
    • Mailed-in requests will be processed within 5 business days.
    • Returned cashier’s checks or money orders will incur a $25 fee.
  • No personal checks are accepted.
